VS2013 build working again:
- Added ICU download and compile steps to bootstrap.sh.
- Switched Qt to shared libraries and enabled ICU and Webkit.
- Switched to shared MS CRT.
- Disable C4244 and C4267 warnings since these are not picked up by GCC.
