Turn counter macros into no-op instructions when counters are disabled.
Evaluating to nothing makes gcc complain when the macro is used in a
conditional. Fixes the following warning:

The copyright and license notice is already in the LICENSE file. There
is no justifiable reason to also require that it be included in every
file, since the individual files are not individually distributed except
as part of the entire package.
Update following macros:
* NODE_NET_SOCKET_{READ,WRITE}() - they both take 4 arguments,
not 2
* NODE_COUNT_GC_PERCENTTIME() - it takes a single argument.
Use INT_PTR instead of INT32 in pointer casts in win32_etw provider.
