This also associates the appropriate internal/errors code with all `assert.AssertionError` instances and updates the appropriate test cases. Anna Henningsen	7922830087	stream: fix `Writable` subclass instanceof checks `2a4b068aca` introduced a regression in where checking `instanceof` would fail for `Writable` subclasses inside the subclass constructor, i.e. before `Writable()` was called. Also, calling `null instanceof Writable` or `undefined instanceof Writable` would fail due to accessing the `_writableState` property of the target object. This fixes these problems. assert: fix AssertionError, assign error code

Using `assert.AssertionError()` without the `new` keyword results
in a non-intuitive error:

The `assert.AssertionError()` can only be used correctly with `new`,
so this converts it into a proper ES6 class that will give an
appropriate error message.

This also associates the appropriate internal/errors code with all
`assert.AssertionError` instances and updates the appropriate test
cases.

stream: fix `Writable` subclass instanceof checks

2a4b068aca introduced a regression in where checking
`instanceof` would fail for `Writable` subclasses inside the
subclass constructor, i.e. before `Writable()` was called.

Also, calling `null instanceof Writable` or
`undefined instanceof Writable` would fail due to accessing the
`_writableState` property of the target object.

This fixes these problems.
