When autocompleting `require` calls, the repl strips .js file extensions
from results. However, stripping an extension from a directory results
in an error. Update the autocompletion logic to avoid stripping
extensions from directories.

test/fixtures/test-error-first-line-offset.js is used in the
test/sequential/test-module-loading.js, but the line break type
is insignificant. It can be replaced for consistency.

`module.children` is supposed to be the list of modules included by this
module but lib/module.js failed to update the list when the included
module was retrieved from `Module._cache`.

This commit contains three separate changes:
- Always return a string from ToUnicode no matter if an error occurred.
- Disable CheckHyphens boolean flag. This flag will soon be enabled in
the URL Standard, but is implemented manually by selectively ignoring
certain errors.
- Enable CheckBidi boolean flag per URL Standard update.
This allows domain names with hyphens at 3 and 4th position, as well as
those with leading and trailing hyphens. They are technically invalid,
but seen in the wild.
Tests are updated and simplified accordingly.

If an asynchronous function is passed no callback function, there is no
way to return the result. This patch throws an error if the callback
passed is not valid or none passed at all.

The file no longer works after the removal of the --debug/--debug-brk
switches in commit 47f8f74 ("src: remove support for --debug".)
This commit also removes several tests that still referenced the
old debugger but were either unit-testing its internals or passing
for the wrong reason (like expecting an operation to fail, which
it did because the debugger is gone.)

On Windows when REPL history file has the hidden attribute node will
fail when trying to open it in 'w' mode. This changes the mode to
'r+'. The file is guaranteed to exists because of earlier open call
with 'a+'.

Adds support for the PSS padding scheme. Until now, the sign/verify
functions used the old EVP_Sign*/EVP_Verify* OpenSSL API, making it
impossible to change the padding scheme. Fixed by first computing the
message digest and then signing/verifying with a custom EVP_PKEY_CTX,
allowing us to specify options such as the padding scheme and the PSS
salt length.

0-dns-cert.pem and 0-dns-key.pem were stored in `test/fixtures/key`
directory, but the cert file cannot be created with the openssl
command via Makefile.
Added a script to create it with using `asn1.js` and
`asn1.js-rfc5280` and moved them out of key directory and put into
`test/fixtures/0-dns`.
The domains listed in the cert were also changed into example.com and
example.org to show the use for only testing.

Currently when node is build --without-ssl and the test are run,
there are a number of failing test due to tests expecting crypto
support to be available. This commit fixes fixes the failure and
instead skips the tests that expect crypto to be available.

This commit implements the Web IDL USVString conversion, which mandates
all unpaired Unicode surrogates be turned into U+FFFD REPLACEMENT
CHARACTER. It also disallows Symbols to be used as USVString per spec.
Certain functions call into C++ methods in the binding that use the
Utf8Value class to access string arguments. Utf8Value already does the
normalization using V8's String::Write, so in those cases, instead of
doing the full USVString normalization, only a symbol check is done
(`'' + val`, which uses ES's ToString, versus `String()` which has
special provisions for symbols).
