This also associates the appropriate internal/errors code with all `assert.AssertionError` instances and updates the appropriate test cases. assert: fix AssertionError, assign error code

Using `assert.AssertionError()` without the `new` keyword results
in a non-intuitive error:

```js
> assert.AssertionError({})
