Enable `space-unary-ops` in `.eslintrc`. This prohibits things like:
i ++ // use `i++` instead
typeof(foo) // use `typeof foo` or `typeof (foo)` instead

Currently there are many instances where assert.fail is directly passed
to a callback for error handling. Unfortunately this will swallow the
error as it is the third argument of assert.fail that sets the message
not the first.
This commit adds a new function to test/common.js that simply wraps
assert.fail and calls it with the provided message.
Tip of the hat to @trott for pointing me in the direction of this.

Enable linting for the test directory. A number of changes was made so
all tests conform the current rules used by lib and src directories. The
only exception for tests is that unreachable (dead) code is allowed.
test-fs-non-number-arguments-throw had to be excluded from the changes
because of a weird issue on Windows CI.

Fix a race condition in parallel/test-vm-debug-context where the 'exit'
event for the child process is emitted before the first and only 'data'
event for the child process's stderr stream.
I considered deferring the 'exit' event in lib/child_process.js until
all stdio streams have been closed but I realized that's not going to
work when the child process spins off grandchildren that keep the stdio
file descriptors alive.

It's possible for an accessor or named interceptor to get called with
a different execution context than the one it lives in, see the test
case for an example using the debug API.
This commit fortifies against that by passing the environment as a
data property instead of looking it up through the current context.

Ensure that the debug context has an Environment assigned in case
a fatal error is raised.
The fatal exception handler in node.cc is not equipped to deal with
contexts that don't have one and can't easily be taught that due to
a deficiency in the V8 API: there is no way for the embedder to tell
if the data index is in use.

The copyright and license notice is already in the LICENSE file. There
is no justifiable reason to also require that it be included in every
file, since the individual files are not individually distributed except
as part of the entire package.
Compiles and executes source code in V8's debugger context. Provides
a programmatic way to get access to the debug object by executing:
var Debug = vm.runInDebugContext('Debug');
